You can easily do this in the Sewers in Baldur's Gate. As you go through the sewers, you'll find yourself in a room containing normal barrels (the larger ones) as well as a black powder barrel (smaller but lighter in color) that has three rats trying to reach your face. Simply hit the black powder bar to light the fuse. After that, wait for the explosion to occur. It's possible that the rats will quickly die.

The explanation is obvious and will come naturally depending on how you play. This was something I found out while playing with Borador the dwarf (Borador). I used a crossbow, but bows and throwing knifes should work.

You can unlock Drizzt by completing the game on any difficulty. Once you have Drizzt unlocked, you can select Alessia the cleric and press DOWN on the D-PAD. You can either go a bit beyond the sewer quest to get 500 kills or use Drizzt during your Extreme Mode playthrough.

Artemis Entreri allows you to defeat 500 enemies
 
 
You need to complete the game on extreme difficulty in order to unlock Artemis. Once you have obtained the unlock key, you can select Artemis from the character selection screen by choosing Alessia (the Clerical) and then pressing down twice on your DPAD. If you start a fresh game, you will need to obtain 500 kills.

Important note regarding most of these achievements
 
 
After you've defeated the last boss, you'll be asked if it is okay to save the game. You can save your game to help you get the most out of the playthrough achievements.
 
 
After loading the savegame (pause, change players, import or new), you can import any character from an existing savegame or a new level 1. Once the import is complete, click the black pylon and watch the end credits to claim your playthrough achievement.
 
 
This applies to all achievements which require you to play the game with a specific character.

Self-explanatory. I wouldn’t recommend starting a new character to do this. Instead, you can use Drizzt (unlocked by finishing the game on any difficulty) as well as importing the character from your first playthrough. If you import a normal player into extreme mode, they will only be able to keep their skills and levels. Drizzt begins with very good melee weapon so that it makes things easier.

You will need one feat/skill for rank 5 with all standard characters (exclusions Drizzt/Artemis unlockable characters).
 
 
Note: Since I'm not sure how this is being recorded, I decided to play it safe and created 3 savegames containing 3 different characters as well as the 4th savegame from the first playthrough.
 
Use +wisdom gear if you have it to increase your experience gain. By the time you finish the sewers quest, you should be able reach rank 5 with any skill. To make it faster, import your endgame player and drop all gear and high-damage weaponry.
 
Once I had 4 savegames of 4 different characters (each one having at least one skill at rank 5, I started a brand new game with the last one missing, played until I reached rank 5, with one skill, and the achievement popped.

Maximum Fun
 
Max level for any character
 
 
You must reach level 40 to get this achievement, which is not usually possible in your first playthrough. This can be obtained by using Drizzt or an import of a character. This will be faster if you have items which increase your experience gain with +wisdom.
 
 
Alternatively:
 
Sidequests allow you pay gold for experience in towns for the cleric or dwarf, monk, or necromancer. These quests require you to pay 2000 gold for 2000 experience. You can do them four times, with each increase in experience and price. The first quest costs 2000. Second, 4000, Third, 8000, and Fourth, 16000.
 
You can make these quests multiple times by using two savegames (let’s just call them A andB) and the character-import feature. Start with savegame "A" where you haven’t yet done the quests. (Or at least not the 16000-gold one). Then import character from Savegame B. Save the game as savegameB after you complete the quest. Load savegame AB again and import your character from savegame B. Next, spend 16000 Gold again, then save as savegame AB. Rinse and repeat.
 
If you are short of gold, import a character with a high-value gear, drop it, and then sell it as often you need.

Crafting All Day
 
42 custom items can be created in the Workshop
 
 
A base item (no +X prefix required) of at minimum fine quality is necessary to craft items. Fine Crossbow (not Crossbow and Shoddy Crossbow!) as well as 1 Rune Stone. Rune Stones can either be purchased from a vendor in town, or you can find them during a playthrough. Additionally, vendors sometimes sell gear of better quality. After you have the items in your possession, visit the vendor at Baldur's Gate. Ask to use his workshop and then combine the item and the runestone.
 
You can import characters to drop the items, without having to purchase or find them 42 more times. To be clear, you don’t need to craft 42 unique items. However, you can always craft one (e.g. crafting a +1 Fine Crossbow).
